Volume loss is one of the harder things to point to in a mirror. Nothing has changed exactly, and yet the cheeks sit lower than they did, the temples have hollowed slightly, and the face looks tired in photographs taken on days you felt fine. Filler is the usual answer, and it is an excellent one, but not everyone wants a product placed in their face. PRFM is the alternative we reach for most often in that situation: a filler made entirely from you.

What PRFM is

PRFM begins the same way PRF does, with a small sample drawn in our office and spun slowly to preserve your platelets, growth factors and the natural fibrin that holds them. It is then gently heated, which transforms the material into a firmer, more structured gel. That added density is the whole point. Where PRF is fluid enough to be applied to the skin or placed lightly beneath it, PRFM has the substance to hold shape and restore contour, which is why we use it as a filler and not as a skin booster.

How PRFM restores volume

A hyaluronic acid filler occupies space from the moment it is placed. PRFM works differently, and more slowly. It provides gentle immediate fullness while the growth factors held within it prompt your own tissue to build new collagen, so the volume that develops over the weeks that follow is genuinely yours. The effect is subtle by nature. It is not the instrument for a dramatic contour change, and we would not pretend otherwise, but for patients who want a natural alternative to traditional fillers.

Where we use it

PRFM is used across the face where softening has occurred and structure is wanted: the cheeks, the temples, the jawline and the folds that deepen from the nose to the mouth. We do not use PRFM beneath the eyes. That skin is exceptionally thin and demands a lighter material, so when the under-eye area is the concern we use PRF or a hyaluronic acid filler chosen for the purpose instead.

PRFM, PRF and PRP

All three begin with the same small sample and differ in what is done with it afterward. PRP is the longest established, spun quickly to concentrate the platelets, and remains the treatment we perform most often. PRF is spun more slowly to preserve the fibrin, which lets it release growth factors gradually over several days; it is fluid, and we use it as a skin booster, as a subtle filler and on the scalp. Your treatment

Your appointment begins with a small sample drawn in our office. While it is prepared, the treatment areas are numbed. The PRFM is then placed where volume is wanted, much as a filler treatment would be. Most visits take roughly an hour. Mild swelling or bruising is possible for several days afterward, and your provider will review aftercare with you before you leave. Because the volume develops as your own collagen does, we recommend a series rather than a single treatment, and we will suggest a sequence based on what we find.

Is PRFM right for you?

PRFM suits patients who want volume restored without a product introduced, who are content for the result to arrive gradually, and whose goal is to look rested rather than noticeably altered. It is a considered choice for cheeks and temples that have softened, and for patients who have declined traditional filler for reasons of preference or sensitivity.
